First Day of School Needs
You don’t need anything for the first day. Supplies such as pencils, crayons, paper, and other items are available to all students. Teachers will provide requests for items such as hand sanitizer, napkins, paper towel, and snacks on a classroom by classroom basis. If you can donate, we’d love to have your help!
Field Trip Parent/Guardian Form
If you want to volunteer at Parkwood-Upjohn or attend a field trip with your student, you MUST have a signed Record Check Form (el formulario en español) on file at the office. This form MUST be updated annually. Please fill out this form before attending the field trip with the classroom.
